Care Manager (RN)

Centene is seeking a remote Care Manager (RN) based in California

This role is crucial for developing, assessing, and facilitating complex care management activities primarily for members with physical health needs, with the goal of achieving high-quality, cost-effective healthcare outcomes

The Care Manager will create personalized care plans and provide education for members and their families

Key responsibilities include evaluating member needs, identifying barriers to accessing appropriate care, and addressing social determinants of health

The RN will focus on member-identified priorities, recommending and facilitating plans for optimal outcomes

This involves developing ongoing care and service plans, collaborating with providers to identify specialists and community resources, and providing continuous follow-up and monitoring of member status and progress toward goals

The role requires strong problem-solving skills to identify and intervene on barriers to care and to coordinate effectively with the member, caregivers, and the care provider team
