Care Manager, Telephonic Nurse

Humana is hiring a full-time, remote Care Manager, Telephonic Nurse to serve as the primary coordinator and point of contact for beneficiaries across the medical and behavioral health spectrum

This role involves utilizing clinical skills to make a significant impact on patient care by coordinating and collaborating with other members of the care management team or external programs to ensure a fully integrated care plan

Responsibilities include assessing beneficiary needs, developing comprehensive care plans, coordinating services, and educating beneficiaries on their health and available resources

The Care Manager will use their expertise to guide patients through complex healthcare decisions, addressing all beneficiary needs and conditions

The position requires strong communication skills and the ability to work effectively in a remote environment, managing a caseload of patients telephonically
