Pharmacist, Clinical/Medication Therapy Management (Remote PST)

Molina Healthcare is seeking a Clinical Pharmacist for a remote Medication Therapy Management (MTM) role

This position is crucial for ensuring members have access to necessary prescription drugs while promoting safe and cost-effective medication use, adhering to federal and state laws

Key duties include conducting telephonic targeted and comprehensive medication reviews, meticulously analyzing medication profiles to optimize patient regimens, and proactively identifying potential patient safety issues, such as drug interactions or suboptimal medication regimens

The pharmacist will promote evidence-based and clinically appropriate prescribing practices through various modalities, perform prior authorization reviews for high-cost medications, and develop and implement drug utilization management strategies

Collaboration with members, providers, and care teams is essential for coordinated care

Candidates must possess a Doctor of Pharmacy (Pharm.D.) degree or a Bachelor's Degree in Pharmacy with 10 years of relevant clinical experience, an active and unrestricted State Pharmacy License, and an ASHP-accredited residency or 2-3 years of relevant pharmacy experience

Current knowledge in clinical pharmacology and disease management is also required.
