Virtual Clinical Pharmacist - 25-08

This full-time Virtual Clinical Pharmacist role supports the development and implementation of programs, policies, and procedures focused on medication optimization and high-quality disease state management

Key responsibilities include patient outreach for medication management in chronic diseases, collaborating with physicians for longitudinal patient care, and optimizing interventions for best patient outcomes

The role actively contributes to quality initiatives, performing patient and provider outreaches to close quality gaps, and conducting comprehensive medication reviews for high-risk or non-adherent patients

You will also participate in virtual care team rounds and evaluate patients on specialty drugs for regimen optimization

A Pharm.D. degree and a valid California Pharmacy license are required, with PGY-1/Residency experience or equivalent preferred

The position requires strong attention to detail, organizational skills, excellent communication, and the ability to travel within Northern California occasionally.
