This Ambulatory Care Pharmacist position offers a flexible work arrangement, allowing for work from home while requiring some onsite presence to connect with colleagues and leadership
The role is responsible for delivering comprehensive pharmaceutical care within an ambulatory setting, focusing on medication management, patient counseling, and collaboration with multidisciplinary teams
Responsibilities include reviewing prescriptions for appropriateness, identifying potential drug interactions, and providing patient education to optimize medication adherence and therapeutic outcomes
The pharmacist will also contribute to quality improvement initiatives and ensure compliance with all regulatory standards.

Requirements
- Doctor of Pharmacy (PharmD) and current pharmacist license in good standing.
- 3+ years' experience as a clinical pharmacist.
- Strong clinical foundation and experience in providing clinical pharmacy services.
- Ability to perform phone, virtual, or video-based consultations.
- Proficiency with electronic medical record systems and integrated technology.
- Excellent communication and patient counseling skills.
- Ability to work independently and as part of a collaborative team.
