Case Manager Registered Nurse

This is a fully remote, full-time RN Case Manager position with CVS Health, open to candidates from any state, with a preference for those in compact RN states

The role focuses on telephonically assessing, planning, implementing, and coordinating case management activities to evaluate members' medical needs and facilitate overall wellness

Key responsibilities include developing proactive care plans to enhance short and long-term outcomes, utilizing clinical tools and data to evaluate member needs and benefit eligibility, and applying clinical judgment to address complex health and social indicators that impact care planning

The Case Manager will review prior claims, assess work capacity, and consult with supervisors to overcome barriers

Strong motivational interviewing skills are essential for maximizing member engagement and discerning health status and needs

A dedicated, distraction-free home workspace and high-speed internet are required.
