Telephonic Nurse Care Manager

This full-time, remote Telephonic Nurse Care Manager position with Advanced Monitored Caregiving Inc. is based in Austin, TX, and requires strong clinical judgment, excellent communication skills, and comfort working in a technology-enabled, fast-paced virtual care environment

Candidates should possess a minimum of 5 years of broad clinical nursing experience, with at least 3 years specifically in care management, care coordination, home health, transitions of care, telehealth, virtual care, or chronic disease management (e.g., cardiac, pulmonary, endocrine)

The role involves providing comprehensive telephonic care management, utilizing clinical expertise to assess patient needs, develop personalized care plans, and educate patients on effectively managing their health conditions

Strong computer proficiency, including efficient EHR documentation and navigating multiple systems, is essential for success in this role.
