Case Manager Registered Nurse - Fully Remote

This fully remote Case Manager RN position involves providing telephonic and/or face-to-face case management activities for members

Responsibilities include assessing, planning, implementing, and coordinating care to evaluate medical needs and facilitate overall wellness

The RN Case Manager develops proactive courses of action to address member issues, enhances short- and long-term outcomes, and integrates strategies to reduce risk factors

Utilizing clinical tools and data review, the role conducts comprehensive evaluations of member needs, benefit plan eligibility, and facilitates smooth transitions to Aetna programs

The position requires applying clinical judgment to address complex health and social indicators, which impact care planning and resolution of member issues

Assessments will consider information from various sources, including co-morbid and multiple diagnoses, to ensure comprehensive care

Candidates can reside in any state, with a preference for those in compact RN states, and CVS Health supports the cost of required licensure

Normal working hours are Monday-Friday, 8:00 am - 6:00 pm in the time zone of residence, with some late shift rotation and holiday on-call as needed.
