Registered Nurse Care Manager (RNCM)

Heritage Health Network is seeking a Registered Nurse Care Manager (RNCM) to provide essential clinical oversight to a team of non-clinical Lead Care Managers

In this fully remote role, the RNCM will be responsible for reviewing member charts within the electronic medical record (EMR) system, meticulously assessing existing care plans, and actively collaborating with Lead Care Managers

The primary goal is to ensure that all members receive comprehensive, high-quality clinical care tailored to their individual needs

This role emphasizes a person-centered approach, considering the full spectrum of a member's life context to improve health outcomes

The position offers flexibility, with most hours able to be completed at the RN's convenience

The RNCM will play a critical role in bridging healthcare accessibility gaps and facilitating empathetic healthcare solutions, particularly within diverse communities.
